Please can you supply me with your original press release, and the original source of text for your press release you issued in 2008 to Surrey newspapers about a new HPV vaccination programme (Cervarix) for 12 year old girls in schools, and a catch-up programme for teenage girls, e.g.

The reason I ask, is that apparently, you informed the public that "The vaccine itself is safe, with no serious side effects reported either in clinical trials or among the hundreds of thousands of women who have received it worldwide."
And yet there is EMEA documented evidence that serious side effects were indeed reported in Glaxo Smith Kline clinical trials - and among the millions of girls and women who have received it worldwide. There is further evidence via MHRA and VAERs. Even the manufacturers have refused to comment to me on the risks of serious side effects, such as risks to fertility, and risks to girls who already have a medical condition.
For your information, the Department of Health admitted to me in October 2008 that they published and printed 50,000 copies of a 32-page booklet with false information about side effects of the HPV vaccines. I found this was still online in June 2009 so I freely gave all the details to the Department of Health at a Ministerial request, of which they have just denied receipt. There is plenty of local evidence in Surrey and across the UK that those serious side effects can be long-term. Perhaps this great cost to the public purse was not anticipated or budgeted for, in Health, Education budgets etc..

Please could you let me know the print run figures and dates for the original document "Beating cervical cancer - the facts", reference number 288939_HPV_Factsheet.pdf.
If any of them were recalled, how many?
Please would you also inform me of the date of any print runs which have included the revised safety information, as per my FOI question dated 1st October 08 which pointed out the error in the statistics. The reference number online became 288939_HPV_Factsheet2.pdf.
Could you also let me know whether an Erratum/Corrigendum message was ever sent out to make the safety information clear and accurate? If so, please inform me of the date, the wording and the number of copies sent out.
This is your corrected wording on page 13: "...and joint pain were reported in less than one in ten but more than one in 100 people. A full list of potential adverse reactions can be found on the..."
The original wording was "...and muscle aches were reported in less than one in 1000 people."

3. Please provide me with the number of incidents of anaphylaxis resulting from vaccinations in the UK during the last 5 years.
4. Please provide me with the number of deaths resulting from vaccinations in the UK during the last 5 years.
